چکیده

Introduction Spontaneous breathing during mechanical ventilation is still controversial. It is well known that incongruous patterns of mechanical ventilation can trigger or exacerbate lung injury. NAVA is a mode of mechanical ventilation that delivers ventilatory assist in proportion to the Electrical activity of the Diaphragm (EDi), which mirrors the performance of the whole respiratory system. However during NAVA, the effects of PEEP on the EDi signal, respiratory rate and breathing heterogeneity are unknown.

Physiologic response to changing positive end-expiratory pressure during neurally adjusted ventilatory assist in sedated, critically ill adults.

BACKGROUND Neurally adjusted ventilatory assist (NAVA) delivers airway pressure (Paw) in proportion to neural inspiratory drive as reflected by electrical activity of the diaphragm (EAdi). Changing positive end-expiratory pressure (PEEP) impacts respiratory muscle load and function and, hence, EAdi. We aimed to evaluate how PEEP affects the breathing pattern and neuroventilatory efficiency duri...

Feasibility of neurally adjusted positive end-expiratory pressure in rabbits with early experimental lung injury

BACKGROUND During conventional Neurally Adjusted Ventilatory Assist (NAVA), the electrical activity of the diaphragm (EAdi) is used for triggering and cycling-off inspiratory assist, with a fixed PEEP (so called "Triggered Neurally Adjusted Ventilatory Assist" or "tNAVA"). However, significant post-inspiratory activity of the diaphragm can occur, believed to play a role in maintaining end-expir...

Clinical assessment of auto-positive end-expiratory pressure by diaphragmatic electrical activity during pressure support and neurally adjusted ventilatory assist.

BACKGROUND Auto-positive end-expiratory pressure (auto-PEEP) may substantially increase the inspiratory effort during assisted mechanical ventilation. Purpose of this study was to assess whether the electrical activity of the diaphragm (EAdi) signal can be reliably used to estimate auto-PEEP in patients undergoing pressure support ventilation and neurally adjusted ventilatory assist (NAVA) and ...

Effects of pressure-support ventilation with different levels of positive end-expiratory in a mild model of acute respiratory distress syndrome

Introduction Pressure-support ventilation improves lung mechanics, blood gas exchange, hemodynamics, and work of breathing (WOB) in mild acute respiratory distress syndrome (ARDS) [1,2]. Nevertheless, those beneficial effects could be dependent of positive end-expiratory pressure (PEEP) applied during mechanical ventilation. So far, no study has compared pressure-support ventilation (PSV) with ...

Diaphragmatic electrical activity: a new tool to assess lung hyperinflation?

447 September 2014 “ EXPIRATION and slow decrease of light together reaching minimum together ... in about 10 s and immediately cry as before. Silence and hold for about 5 s.” In the script for his late play “Breath,” Samuel Beckett conveys in sound and light his awareness of long and vital expiratory times. His personal experience with emphysema might have provided some of the inspiration. In ...
